The Republic of Indonesia Health System Review 2017

Indonesia is in the midst of a series of transitions, ranging from demographic and epidemiological, to social, economic and political. After decades of authoritarian and centralized government, Indonesia introduced reforms in 1998 to establish stable democratic government, with significant devolution of authority to provincial and district levels of government. Strong economic growth is leading the country towards middle-income status. However, government investment in the health system has been limited, leading to insufficient facilities and workforce needed for public services, and encouraging the growth of private health facilities. Problems of maternal and child health, nutrition and communicable diseases persist, while noncommunicable and chronic diseases are emerging as new priorities. There are significant regional disparities in terms of health status and in the quality, availability and capacity of health services. Decentralization has affected the capacity of the central Ministry of Health to maintain integration and alignment across the different levels of the health system. Government investment in health has increased since the economic downturn in 1997 with the increasing priority to create ‘social safety nets’ in the form of social health insurance programmes for the poor, culminating in the establishment of a universal social health insurance scheme (Jaminan Kesehatan Nasional or JKN) in 2014. The challenge for the government is to expand this scheme to achieve universal health coverage by 2019, while addressing regional disparities in service quality and accessibility, managing resources effectively, containing costs and minimizing fraud, engaging the private sector, and maintaining investment in health promotion and prevention programmes.

Locus of Control Merestorasi Terumbu Karang pada Masyarakat di Pulau Bontosua, Kepulauan Spermonde, Provinsi Sulawesi Selatan

Penelitian ini mengkaji dan menganalisis faktor locus of control yang mempengaruhi perilaku merestorasi terumbu karang pada masyarakat di Pulau Bontosua Kepulauan Spermonde Provinsi Sulawesi Selatan. Pemilihan lokasi ini didasarkan pada kondisi kerusakan ekosistem terumbu karang, adanya program restorasi ekosistem terumbu karang yang dilaksanakan melibatkan masyarakat, serta meskipun letaknya di pulau-pulau kecil, namun sarana transportasi dan komunikasi lancar. Pulau Bontosua secara administratif masuk ke dalam Kecamatan Liukang Tupabiring Kabupaten Pangkajene Kepulauan. Penelitian dilakukan pada bulan Agustus sampai November 2022. Penelitian ini dilakukan menggunakan perspektif paradigma kuantitatif, penelitian survei dan ex post facto. Pengambilan sampel dilakukan secara acak berstrata (stratified random sampling). Jumlah sampel penelitian sebanyak 72 orang. Teknik pengumpulan data yang digunakan dalam penelitian ini yaitu kepustakaan atau referensi literatur, observasi, kuesioner, wawancara atau interview, serta dokumentasi. Tingkat locus of control pada masyarakat berada pada kategori rendah. Dalam penelitian ini ditemukan bahwa locus of control merestorasi terumbu karang pada masyarakat lebih ditentukan oleh faktor orang lain dibandingkan oleh kemampuan dirinya sendiri, maupun ditentukan oleh nasib, peluang dan keberuntungan. Strategi untuk meningkatkan locus of control merestorasi terumbu karang pada masyarakat di antaranya fasilitasi modernisasi alat tangkap perikanan, pengembangan dan diversifikasi usaha pada masyarakat pesisir, serta penguatan karakter ekologis melalui pelaksanaan kegiatan keagamaan.

Problems related to acute respiratory infection among under-5 children in Sorong, West Papua: a community diagnosis approach

Acute respiratory infections (ARIs) are the leading cause of morbidity and mortality in children under five years, with a periodic prevalence of 25.9% in West Papua (2013). This study aims to explain the factors associated with ARIs in under-5 children in Sorong District, West Papua. This cross-sectional study was conducted in two districts, where trained medical students interviewed 135 mothers/caregivers by using a structured questionnaire and interviewed cadres and health workers for qualitative observation. About 85.5% of respondents stated that their children had ARIs in the previous two weeks (n=135), much higher than the data from Sorong District Health Office, which only reached 24.63%. Dominant risk factors were large household size, smoking at home, and improper handwashing habits. Only half of the respondents went to a physician in a primary care facility. This paper explores the acceptability of a public-private partnership program on the provision of subsidized artemisinin-based combination therapies (ACTs) in community pharmacies from the perspectives of private health providers, patients, and program implementers. Research on mangrove vegetation on Rowsar Island, in the area of the National Park of Cenderawasih Bay, This research was conducted from March to April 20223. West Papua Province conducted to identify and understand the condition of the mangrove forest. This research use quantitative methods by inventory with path plots. Based on the observations of mangrove vegetation on Roswar Island, 10 types of true mangroves identified, i.e Rhizopora mucronata, Rhizopora apiculata, Sonneratia alba, Xylocarpus granatum, Sonneratia caseolaris, Bruguiera gymnorrhiza, Avicennia marina, Avicennia rumphiana, Aegialitis annulata, dan Aegiceras floridum. The general mangroves found at every station are Rhizopora apiculata dan Rhizopora mucronata. Rhizopora apiculata grows in muddy, soft, deep, and waterlogged soil.
